Medical Records

Patients must sign a standard “Medical Release” authorization form. There is no charge for medical records sent to another medical facility. If you want to obtain your medical record you will be charged a minimal fee per page, postage and handling fee.

Please be advised that N.J.A.C. 8:43A-13.5 determines the copy fee for medical records. It states that the fee for copying records shall not exceed $100 for the first 100 pages, with a $0.25 charge for each page thereafter, and the fee for the entire record shall not exceed $200.00. Additionally, there is a provision for a $10.00 search fee, as well as a charge for actual postage, up to a maximum of $5.00. The Center requires 72 hours’ notice, and has 30 days to honor a request.
